Please fill out student information and participation waiver below (required).  You only need to do this once per year or if any information has changed.  If you have already filled out this form this year you may skip this process. Also please review checklist and other information at the bottom of this page...thx!

Student Information and Participation Waiver
I, the undersigned, give permission for my child (children) to participate in Smyrna Tennis Academy programs.  I also understand that IN TENNIS and it's instructors, Tolleson Park, The City of Smyrna are not responsible for any injuries that may occur.  I also understand that payments are non-refundable without 14 days notice prior to the start of a given program.

Summer Camp Information

  • All camps run Monday-Thursday (except first week-Tues-Friday).  Friday is reserved as a rain-makeup day.  If it rains more than 1 day in a camp week a makeup day in a future camp will be available to you.

  • Each student will need: Racket, Water, Athletic Shoes

  • When you arrive:

  • Students will meet on court 1 for warm-up and group assignments.

  • Please bring all equipment on court.

  • All tennis camps are subject to cancellation if camp minimums are not met.  You will be notified no less than 3 weeks if this is the case and you will be refunded in full.

  • All FULL day camp students are to bring their own lunch.

  • When Class is over:

  • Each student will receive a piece of candy (if they work hard), please let us know if you do not want your student to have candy.

  • Students are dismissed one at a time.  The parking lot is cramped and visibility is not ideal; for safety please meet your student at the entrance gate to Court 1 and Court 2 for pickup.

  • In the event of bad weather an email will be sent no later than the hour before class begins (please add: chris@smyrnatennis.com to your contacts to insure inbox deliver)

  • If a student misses a day due to illness or otherwise, they are invited to join another class during the summer, please email to schedule.

  • Cancellations or date changes will not be accepted without 2 weeks notice.

School Year Programs

  • We do not have a scheduled restroom break, please take care of this before class begins--facilities are on site.

  • When Class is over:

  • Each student will receive a piece of candy (if they work hard), please let us know if you do not want your student to have candy.

  • Students are dismissed one at a time.  The parking lot is cramped and visibility is not ideal; for safety please meet your student at the entrance gate to Court 1 and Court 2 for pickup.

  • During the winter months, the water fountain is turned off to avoid freezing pipes, please bring water with you.
